Transitional Case/Care Management
Homeless Veterans
Edward Hines, Jr. VA Hospital's Homeless Veteran Supported Employment Program (HVSEP) provides vocational assistance, job development, and placement, and ongoing supports to improve employment outcomes among homeless veterans and veterans at-risk of homelessness. They also offer Vocational Rehabilitation Services a case management service for veterans interested in employment, vocational training, or educational goals.
In partnership with Hines VA Hospital, the goal of this program is to transition veterans experiencing homelessness to stable, long-term housing. When entering the program, participants are placed in a furnished apartment. Through case management and wrap-around services, we work to build the skills they need to maintain the housing independently after completing the program.
Catholic Charities offers two residential communities for Veterans in Cook County. Bishop Goedert Residence is an affordable senior housing property with 70 one-bedroom apartments located on the Veterans Affairs Hines Campus. St. Leo Residence is a permanent supportive housing program that consists of 141 furnished studio apartments.
