Heartland Alliance - Uptown Health Center offers services that target adults who are formerly homeless or at risk of homelessness and who suffer from a serious mental illness. Many of the individuals are people who have achieved improved stability and no longer need a higher level of care. Services include comprehensive assessment, individualized treatment planning, psychiatric and nursing services, mental health counseling and therapy, group counseling and therapy, transportation, crisis intervention, case management, and representative payee services.
BEDS Plus Summit Service Center operates a 24/7 facility with 18 beds and two hospital single-bed rooms for individuals in the south and southwest suburbs of Chicago experiencing homelessness. This includes case management, financial assistance, and other wrap-around services to help transition individuals back into permanent housing.
