Adapt America facilitates the coordination of social services in order to access food, housing, or employment.

Thornton Township's General Assistance Social Worker provides support to individuals applying for Social Security Benefits and Public Aid, particularly those facing literacy and other challenges. Additionally, they assist in relocating homeless and abused individuals to safe and stable shelters, make appropriate referrals for clients dealing with substance abuse and mental health issues, and offer guidance in completing complex forms and applications. Lastly, the social worker collaborates with the Salvation Army to assist individuals who may not qualify for other assistance programs in securing a one-time emergency subsidy, distinct from ongoing general assistance.

Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.

Pilsen Food Pantry's Medical Access Closet (MAC) is open Wednesday and Friday mornings to provide pro bono physical therapy services and medical supplies to people unable to afford these services, either due to non-insurance status, therapy waiting lists, or co-payment problems. MAC also provides supplies and food for Chicago Street Medicine, a medical access program for street-based people. Since its inception, at least 500 people have received services through MAC.

The Salvation Army's Pathway of Hope case management program helps families work their way out of poverty. A trained case worker partners with the family to identify barriers to self-sufficiency (unemployment, insecure housing, lack of job skills, lack of childcare, etc.) and ways to address them by connecting with community support organizations and more.

Saint Anthony operates a community wellness program out of several locations. This includes therapeutic family support groups (available on a weekly basis), mental health services (individual, couples, and family), case management, assistance with accessing healthcare (applying for medical cards, food stamps, etc.), and free pregnancy testing.Please note that free pregnancy testing is only available at the North Lawndale and Little Village locations.

The AIDS Foundation of Chicago case management system provides a wide range of services, acting as a broker between clients and services. Their system identifies medical and social service needs and facilitates referrals to meet those needs; assists clients in applying for benefits, including housing, transportation, food and nutrition services; and facilitates access to emergency financial assistance funds in times of crisis. AFC has also invested significantly in a network of providers offering intensive case management and supportive housing options for people living with a wide range of chronic illnesses.

Facing Forward partners with Chicago Public Schools and University of Chicago Urban Labs to provide housing-focused case management to help students and their families who are experiencing homelessness or housing instability. Our Family Empowerment Team provides additional targeted services to promote healthy development and academic success for children and youth in each household.

The Department of Family and Support Services (DFSS) provides city of Chicago residents with case management and public benefits enrollment through its six Community Services Centers. DFSS also funds private agencies to provide a range of support services including employment training and placement, education, substance abuse treatment referrals, mental health care, domestic violence assistance and more. Other services include provision of clothing and referrals to local food pantries, assistance with Regional Transportation Authority Ride Free Program, and assistance accessing 311's shelter intake system.

Clients collaborate with case managers on service plans that address health goals and assist with applying for benefits, accessing emergency funds, identifying medical and social service needs, and facilitating appropriate referrals.

Hanul Family Alliance provides the Immigrant Family Resource Program (IFRP) which conducts outreach that informs immigrants about the availability of public benefits and other services that support personal and family well-being and economic self-sufficiency. IFRP also provides case management and referrals, language interpretation services, and advocacy.

CCAC provides adults and children in North Lawndale, East and West Garfield Park and Near West Side with a healing space for community-based counseling. They support individuals struggling with depression, trauma, anxiety, impulsive conduct, and anger management.Services include anger management courses, case management, individual/family/couples therapy, trauma-related therapy, and substance use services. They also provide referral services and psychiatric services through their Rush University Medical Center Partnership.

KAN-WIN provides a mentoring program for children who grow up in violent homes. KAN-WIN considers a child's bi-cultural upbringing and provides a culturally sensitive and safe space for children to address their experiences of violence and assist them with developing the skills to cope with negative feelings and resolve conflicts without using violence.

Street-Based Counselors provide clinical case management to individuals returning to the Little Village community after having had contact with the criminal or juvenile justice system, and/or individuals considered high-risk by street outreach workers. Counselors support participants' mental healthcare by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress based on mutually agreed upon goals; and facilitating interdisciplinary approaches.

The Linkage to Care (LTC) team will guide you through the healthcare system to help you get the resources you need to thrive with HIV. Assistance may include accompaniment to appointments, benefit applications, behavioral health screening and referrals, medication assistance and more. The LTC program works with clients who are newly diagnosed with HIV (within the last 90 days) or those clients who have fallen out of care.

Case managers offer compassionate, supportive service during appointments, where they help adults get ID documents, create budgets, update resumes, get health or legal referrals, or apply for employment.

CEDA - Department of Family Support and Community Engagement (FSACE) - West Region provides short-term case management services to families seeking financial assistance through other FSACE programs. Case management includes an assessment of family needs and assists in the development of the Family Action Plan, information and referral, financial literacy, nutrition education and enrollment in other FSACE, agency, and community programs. Clients may be eligible for financial assistance to address other barriers. Any financial assistance will be paid directly to the approved provider.

TCA Health provides various support services, including: health insurance enrollment along with assistance with enrolling in TANF, WIC & SNAP. A Care Coordinators team also helps to bridge the gap between the patient and service providers and can help facilitate appropriate services, schedule appointments, assist with community resources, and encourage health promotion through patient education.

Thresholds' Youth & Young Adult Services (YAYAS) offer groundbreaking programs for adolescents, young adults, families, young mothers, and their children. These programs offer the development skills and support they need to master mental health challenges while reducing the negative impact of those challenges on what matters most to young people: relationships, school success, recreational activities, health and wellness, day-to-day living, and early career experiences.
