Saint Leonard's Ministries provides supportive transitional housing for formerly incarcerated men and women through Saint Leonard's House, Grace House (women only), Saint Andrew's Court, and Harvest Commons Residence. This includes case management, benefits application assistance, AA groups, NA groups, relapse prevention, spiritual enrichment, and permanent housing search assistance.

The purpose of is program is to inform those in the Polish community with limited English skills about services and eligibility requirements of IDHS programs. The YASS Program is a community-based case management program. The purpose of the program is to support young people experiencing homelessness around issues related to housing, mental health, substance and alcohol use, medical care, and employment. The YASS Program provides intensive individualized case management services over the course of 9 months. For these 9 months, a participant will be a part of a team of support with the goal of connecting you to long-term stable housing, employment, and other community resources.

MAC also provides supplies and food for Chicago Street Medicine, a medical access program for street-based people. Since its inception, at least 500 people have received services through MAC.

The program connects clients from the moment they leave jail or prison.
