Provides lead screening for children. All high risk infants receive home visits and follow-up as needed.
Assesses children at risk for lead poisoning and provides referrals as needed.
For children that have elevated blood levels, the health department will work with families to determine the source of lead in their environment. Medical case management staff will support and work with the families to address the lead source and lower the level of lead in the blood.
Assists individuals and families with making informed decisions regarding their health and the health of their family. Services provided: - Family planning counseling and referral. - Pregnancy testing. - Prenatal counseling and follow-up. - Referral for medical card. - High risk infant follow-up. - Family case management. - Lead poisoning testing and counseling. - WIC (Women, Infants, and Children Program). Pediatric Developmental Screening: Provides evaluation and assessment to determine whether there are developmental delays or other issues that need to be addressed. WIC: Provides healthy food, nutrition education and counseling, and breastfeeding support for pregnant and breastfeeding individuals, infants, and children. Also provides referrals for health care and other services. Family Case Management: Provides family case management services to pregnant individuals, infants, and children with high-risk medical conditions. Offers assistance with: - Finding a doctor for prenatal care. - Finding a pediatric doctor. - Understanding proper nutrition. - Understanding the stages of child development. - Getting information on how to become a better parent. - Understanding the importance of prenatal care. - Learning the signs and symptoms of 0-term labor. - Understanding the importance of regularly scheduled well child visits. - Understanding the importance of immunizations. Family Case Management can also help connect individuals with other resources, including: - WIC. - Family Planning. - Health Education. - Hearing/vision. - Substance abuse. - Prenatal/parenting classes. - Child care. - Housing. - Smoking cessation. - Lead screening. - Transportation. - Early intervention. - Immunizations. - Well child exams. - All Kids (health insurance). - Domestic violence.
