- Forms available by mail, on website, or in office - Individuals eligible for Illinois Community Care Program or Illinois Division of Rehabilitation Services must apply through case manager, St. John's staff available to help as needed

- $13.50/hour, four-hour minimum - Some partial scholarship assistance available - call for details

Eligibility

- Adults age 18 years and older who are challenged by physical or mental disabilities - Must be able to help transfer self to chair and must not be a danger to others - Anyone in need unless they require lifting, constant one-on-one care, or are a danger to staff or other participants

Provides advocacy, education, and outreach to individuals challenged by aging or disabilities living in the Metro-East community. Services include Medical Equipment/Supplies, Adult Day Program Centers, Caregiver/Care Receiver Support Groups, Health/Disability Related Support Groups, Daily Living Aid Donation Programs, Walking Aid Donation Programs, and Wheelchair Donation Programs.
