Open to ages 64 years and 9 months (3 months before 65th birthday) or disabled if under 64 years of age
Required documents
Medicare card (red, white, and blue), Insurance card(s) (supplemental insurance, and/or prescription drug cards), All prescription drugs in their bottles, or pharmacy print out of all current prescriptions.
