MEDICAL CARE EXPENSE ASSISTANCE *CHILDREN | UNITEDHEALTHCARE CHILDREN'S FOUNDATION
- Provides medical grants to help children, 16 years old and younger, gain access to health-related services not covered, or not fully covered, by their family’s commercial health insurance plan. - Families can receive up to $5,000 annually per child ($10,000 lifetime maximum per child), and do not need to have insurance through UnitedHealthcare to be eligible.

- Resident of the United States. - Covered by a commercial health benefit plan (excludes Medicaid, Medicare, or any other government subsidized health plan). - Financial need of the child's family will be evaluated and documented through information provided on the application and by submission of a photocopy of the most recently filed Federal tax return (Internal Revenue Service 1040, 1040-A, or 1040-EZ). - Other qualifications may apply.
