Two forms of identification, except in case of homeless individual: (a) One must include his/her residence address, (b) Driver's license, social security card, public aid identification card, utility bill, employee or student identification card, lease or contract for a residence, credit card, or a civic, union or professional association membership card, (c) Affidavit of Resident if unable to provide ID with current residence.
