Call for an appointment or visit during program hours Documents Required: - ID’s for both client and their possible Dependent (Can be a Passport, SSN, ITN #, Consular Card, or Birth Certificate) - SNAP or WIC benefits - Proof of Income (Possible) - Student/Child report card (Possible) - Notice of Decision Letter from Medicaid (Possible) - Lease Form (Possible) *Every client case is different and may vary.*
Eligibility
Must meet at least one of the following criteria: - Has a child that receives free or reduced school lunch through the national school lunch program. - Receives any federal benefits such as SNAP, Medicaid, Federal Public Housing Assistance, SSI, WIC, or Lifeline. - Has Income at or below 200% of the Federal Poverty Guidelines. - Received a Federal Pell Grant during the current award year.
