(1) Complete a Child Support Application available from the website, or call the customer service number listed or email to have an application sent to you (2) Fill out and sign the application form and mail it to the regional office (3) An interview will be scheduled after the form has been received

Description

Assistance with establishing paternity, enforcing orders for child support (including health insurance), modifying or changing the amount of the child support order, collecting current and past due payments, and locating the parent who does not live with the child.
