Provides services to adults with intellectual and developmental disabilities. Community Support services begin with a planning process designed to support the client and their family. Long and short-term plans for living in the community are created. The Community Support program helps manage DHS bureaucracy with families. Also finds and connects Personal Support Workers (PSWs) with clients. Also teaches 8-week classes on various life skills. Budget and Benefits: - Weekly budgeting. - Saving plans. - Bill paying. - Medicaid and SNAP. - SSA management (not rep payee). - HACC/SRN Network. Health and Safety: - Filling medications. - Making and going to routine appointments. - Connecting to mental health resources. Life Coordination: - Coordinating a weekly/daily routine. - Grocery shopping. - Tech support. - Back-up/after hours support. HBS Management: - PSW scheduling. - PSW training. - PSW oversight and feedback. - Timesheet management. - Agency-based PSW coordination.
