Description
Jackson Park Hospital provides a unique, personalized, and caring preventive medical and social service program designed with "Seniors in mind." Golden L.I.G.H.T. provides seniors over the age of 55 with health care services such as Primary Care Physician, Internal Medicine, Cardiology, Urology, Endocrinology, Rheumatology, Physical Therapy, Ophthalmology, Podiatry, and Social Service. Participants may receive free transportation to all clinic appointments, discounts on prescriptions, and free prescription delivery.
