Description
PrimeCare Health Community Health Centers are Federally Qualified Health Centers (FQHC) that have behavioral health consultants at all sites who work closely with primary care providers to support patients in making lifestyle changes necessary to attain and maintain good health. Services include programs to effectively manage emotional or behavioral difficulties such as anger, anxiety, depression, stress, and bereavement. There are also support services for behavioral change plans related to smoking cessation, weight loss, alcohol use, exercise, and other lifestyle modifications.
