Description

Provides intensive wraparound case management and support services to help break the cycle of homelessness and crisis among individuals struggling with complex behavioral health needs. The programs aim is to help clients on their journey and reduce their use of the justice system, shelters, and emergency departments.

Possible services and supports provided include peer support, recovery services, case management, medication assistance, psychiatric services, psychoeducation, court liaison, therapy, advocacy, employment support, and crisis intervention.
