FAMILY HEALTH SERVICES | PERRY COUNTY HEALTH DEPARTMENT
Provides a variety of Family Health Services such as WIC, APORS, Breastfeeding Peer Counseling, Family Case Management, and HealthWorks. WIC provides: - Supplemental nutritious foods. - Nutrition education and counseling. - Screening and referrals to other health and social services. APORS: Provides case management for families caring for infants with special needs. Follow up services provided as well. Breastfeeding Peer Counseling: Provides breastfeeding counseling for new mothers beginning to breastfeed. Family Case Management: Provides referrals and linkage to care for families. HealthWorks: Provides medical case management for foster children. Provides: - Well child visits. - Referrals. - Follow up with specialists as needed.

Eligibility
WIC: - Pregnant individuals. - Parents/guardians with infants or children up to age 5. - Breastfeeding mothers. APORS: Birth to 2 years. Breastfeeding Peer Counseling: Breastfeeding parents and pregnant individuals. Family Case Management: - Meets income requirements. - Pregnant individuals and infants up to 1 year. HealthWorks: 6 years and younger in foster care.
