Description
New Choice Intervention empowers and promotes the wellness of clients through effective day-to-day oversight and administration of the client's service plan. Client-centered case management services are designed to help clients and their family members gain access to, and benefit from, available Waiver and Medicaid State Plan services, including medical, social, and educational services within the community. Effective referral linkage and support services are provided to promote a client's independent lifestyle.
