Community Support/Individual Services (CSI)
Heartland Alliance - Uptown Health Center offers services that target adults who are formerly homeless or at risk of homelessness and who suffer from a serious mental illness. Many of the individuals are people who have achieved improved stability and no longer need a higher level of care. Services include comprehensive assessment, individualized treatment planning, psychiatric and nursing services, mental health counseling and therapy, group counseling and therapy, transportation, crisis intervention, case management, and representative payee services.

Application process
Individuals must be 18 or older, must be diagnosed with a serious mental illness, and may have a co-occurring substance use disorder. Participants may be homeless as defined by living in places otherwise uninhabitable, have a history of homelessness, or be at risk of becoming homeless. Intake is done by referral through integrated intake by calling (312) 520-7267.
