Eligibility

Served in the military, navy, or air force and not been dishonorably discharged; Current and former Reserve or National Guard members who were called to active duty and completed their service may also qualify; Veterans who enlisted after September 7, 1980, or started active duty after October 16, 1981; Must have served for 24 continuous months or their entire active duty period.

(1) Visit www.hines.va.gov/patients/eligibility.asp to complete an application (2) Submit the form online, mail it to Hines VA Hospital, Eligibility, 5000 South 5th Avenue, Hines, IL 60141, or bring the form to the hospital. (3) Include all required documentation.
