Appointment RequiredHumana and Cigna-Health are the only insurance types accepted. To qualify a patient must have had more than 2 hospitalizations and/or more than 3 ER visits within the year prior to being referred. Or, the patient must have a diagnosis of multiple uncontrolled morbidities. Important Note: Alegis does not work with cancer patients with active treatment and/or hospice care patients.
