Description

Provides home care services to individuals with a serious illness, regardless of age or stage of disease. Supportive care is provided in-home by a nurse, who works with other healthcare providers. Services can be provided along with healing treatments including nursing, physical therapy, occupational therapy, speech therapy, social services, and home health aides.
