Residents of Schaumburg Township having income at or below 200% of the Federal Poverty Level may be eligible for services. Residents who have dental insurance are not eligible to participate in the program.

Application process

Patients need to be financially screened by their township first. Once qualified, patients will be given the phone number of the dental clinic to make an appointment. Begin the process by contacting (847) 285-4503.

Fees

A $30.00 copay may be charged for adult visits. Children's services are free.
