Application process
Call for an appointment Documents required: - Photo ID with current address - Social Security Cards for all individuals on application - Birth Certificates for all individuals on application - Medical Cards for all individuals on application - Citizenship Papers - Marriage License / Death Certificate - Divorce / Separation Papers including (settlements, alimony / maintenance, child support payments, etc) - Military Service Record - Lease, Rent receipts and or / Mortgage payments for the past 6 months, tax bill - Current Checking and Savings Bank Activity Statements. and Bank Statements for the past 6 months. ***If you do not have a bank - you will need receipts and or money orders that you use to pay your bills - 6 month's worth, If you use a bank card an activity statement / printout from their website. - Credit Union Accounts, Trust Funds, Safety Deposit Box Information and or other securities or bonds - Proof of Income - Pay Stubs for the past 30 days - records of all other income for all individuals on application - Titles and or Registration for ALL Vehicles in your possession, including, Motorcycles, Boats, ATV's - All Utility Bills for the household, current and final notices - All Insurance policies - Physician's statement stating your medical condition, Report of incapacity, determination of disability - Proof that you have applied for SSI or SSDI - Provide documentation of expenditures for the past 30 days - Proof of Hardship that put you in to financial trouble
Eligibility
Open to Antioch Township residents. Must have applied for TANF, SNAP, Medical, AABD, Social Security, and Unemployment.
